Effective Transition Planning for Secondary Students with IEPs
The outcome of this professional development series is to provide secondary special education LEA staff and AEA staff with the tools and knowledge they need to conduct high quality transition planning. Transition activities prepare students with Individual Education Plans (IEP) for moving from high school to adult life successfully. Planning is essential to make the transition effective. Transition Planning activities are essential, the differences in school and adult services for disabled individuals are substantial. Transition planning is required to begin by age 14 at the child's IEP meeting. Transition planning includes the school, home and the community. The IEP team is required to discuss and plan the student's post secondary needs in the area of living, learning and working. The student and the parent are an integral part of this planning and the student's needs must be based on assessment data. Districts will be reimbursed up to $50 per day toward the cost of a substitute teacher. A stipend of $50 will be provided for each three hour evening session.

Guidance will be provided in setting research based standards and goals, collecting valid and reliable data as indicators of success and interpreting data to determine if instructional interventions are closing the achievement gap.
