Due to the nature of Fred’s disability, the team has determined that he will graduate with an IEP diploma through progress monitoring of his IEP goals. Fred will attend school a minimum of 4 years and participate in activities related to his current goals.

Current Status:  Fred is currently in his 3rd year and is making satisfactory progress as measured by progress monitoring and participation in activities related to his current goals.

Graduation Date:  5/2009

Courses and Activities need to pursue PSE and graduation:  The graduation date above represents a minimum of four years of high school and will be revisited each year to determine if more years beyond the usual four is necessary.  Fred’s curriculum stems from the district standards and benchmarks but is presented at a lower level of complexity.  Emphasis will be given to communication, mobility, self-care, career training, and community involvement.  Learning opportunities will be provided at school and in the community to generalize the above mentioned skills.

Example:

Mary needs 309 credits to graduate.

Courses needed for graduation:  60 credits of English, 45 credits in Math, Social Studies, and Science, 5 credits of Health, 24 credits of PE, and 85 credits of elective.

Current States:  As an 8th grader Mary has completed 0 credits towards graduation.

Graduation Date:  5/2012

Courses and activities needed to pursue PSE and graduation:  Will need to complete the above stated 309 credits.  Since she states in her four year plan that she has an interest working with people in the health care field, additional courses in science should be taken.  She should look into making application into the career academy health occupations coursework.  When appropriate she should do job shadows and internships in the health field.  Continued work on self-determination and disability awareness as well as direct instruction in social skills will help her make the transition to high school and beyond.
